The Director of Patient Flow has operational accountability for M Health Fairviews Patient Placement & Patient Transfer Centers while maintaining continuous situational system awareness through the Systems Operations Center. This includes reviewing system capacity and barriers to patient flow as well as all external, inter-hospital, intra-hospital transfers for new patient admissions, direct admissions, and transfer outs across the system. They will ensure all communication and dissemination of system patient flow updates and/or barriers are shared with the appropriate stakeholders and leaders in a timely and accurate manner. They will support execution of the System Operations Center (SOC) strategy to improve efficiencies and patient outcomes. They will implement continuous process improvement to achieve department and system goals.
Manages daily operations of System Operations Center Patient Placement & Patient Transfer Centers
Collaborates with SOC leadership, hospital leadership and frontline teams to support seamless patient flow throughout M Health Fairview system.
Manages operations for patient placement and flow of all external, inter-hospital, intra-hospital transfers for new patient admissions, direct admissions and transfer outs. Ensures patients are placed or have plans for the appropriate level of care.
Maintains situational awareness of system patient flow and potential bottlenecks.
Identifies barriers to patient flow and supports team in mitigating as appropriate.
Demonstrates ability to implement problem-solving mechanisms in both routine and emergency situations.
Demonstrates understanding of nursing process and clinical operations when problem-solving.
Oversees all communication and dissemination of system situation awareness to appropriate stakeholders within the SOC and hospital leaders including requests for patient placements and transfers, physician consultations, and discharges. Ensures communications are complete in a timely and accurate manner.
Oversees all preparation for daily system patient flow huddles and other related meetings.
Ensures implementation and development of standards of practice in patient care/transitions that reflect valid research findings.
Bachelors degree in Nursing
10+ years progressive responsibility in healthcare operations leadership
5+ years of experience with patient transfers and flow
Masters degree in Health Care Administration, Nursing, Human Services, Business Administration, Organizational Leadership or related field or additional post-graduate education Experience
5+ years of experience with patient transfers and flow in academic health system setting
