A Recruitment or Relocation Incentive may be authorized for a Highly Qualified Individual. The Transition and Care Management (TCM) Program Manager oversees all transition and coordination of care activities for Post 9/11 Service members and Veterans (SM/Vs) treated at the facility. Their health care and psychosocial problems and needs are complex and require a high degree of clinical oversight and creative problem solving.
