| Utilizes an interdisciplinary approach to organize, coordinate, monitor, evaluate and modify plans of care, focusing on complex medical and psychosocial needs of the Hospital�s patients and their families. Performs pre-admission screening review of patients. Assesses and plans interdisciplinary plan of care that effectively manages patient along the continuum. Ensures appropriate use of resources and coordinates discharge planning.
Qualifications:
Education:
1. RN, licensed in Massachusetts
2. BSN or commensurate experience
3. Computer literacy in Word, Excel, Outlook
Experience:
1. 5 years clinical experience Required
2. Minimum 1 year Case Management Experience Required
3. Case Management Certification preferred
4. Working Knowledge of InterQual Criteria preferred
5. Outcomes/resources management skills
6. Midas and MediTech Experience preferred
