The Registered Nurse, Consult Management, practices as a registered professional nurse means the full scope of nursing, with or without compensation, that incorporates caring for all patients in all settings, through nursing standards recognized by the Division, and includes all of the following and other activities requiring a like skill level for which the registered professional nurse is properly trained. Responsibilities which include, but are not limited to: Supports Veteran's care via tele-health, secure messaging and telephone triage; Helps Veterans to self manage chronic diseases such as diabetes, hypertension, congestive heart failure, chemical dependency, cancer, dementia, and any other conditions that may warrant additional attention; Triages and applies collaborative team approach in identifying, analyzing, and resolving Veteran's care problems, and develop care goals for resolving or managing these issues; Focuses on wellness and patient education, and uses motivational interviewing techniques to help the Veteran meet his/her goals; Participates in interdisciplinary meetings with Veterans, families and team to facilitate collaboration. Maintain a current knowledge of interdisciplinary resources, programs and services to include community resources as appropriate; Collaborates with services, internal and external, to facilitate care; Initiates and participate in quality improvement activities that result in improved outcomes; Serves as a preceptor or mentor to colleagues to enhance professional growth, development and practice; Will be required to draw labs; Responsible for providing coverage for other Nursing PACT team members. Focuses on management of consults (entering consults, tracking consults, and managing consults) This position will be responsible for entering consults, tracking the, and managing consults. May be managing consults from 6 different CBOCS. May be filling in during COVID-19 clinics. Other duties as assigned.
