This master level clinical social worker is assigned to Specialty Mental Health Service at the Oscar G. Johnson VA Medical Center. The incumbent provides specialty independent mental health services to veterans and families, consultations to other services and to the community, and educations of patients, families, and hospital staff on mental disorders and related treatments. Major Functions: - Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs. - Independently conducts psychosocial assessments and independently provides psychotherapy. - Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team. - Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. - Independently identifies high-risk patients and provides case management services. - Provides crisis intervention services, seeking to address the cause as well as the presenting complaint. - Coordinates family conferences and serves as liaison to family members. - Participates in a rotation of on-call services, including modification of tour of duty to accommodate patient care needs on weekends. - Independently provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plans. - Evaluates own practice through professional peer review, case conferences and other organizational means. - Facilitates referrals based upon Veterans needs and eligibility. - Participates in committee assignments. Work Schedule: Full-time; Monday-Friday 8:00 am-4:30 pm; Position is eligible to be included in possible evening and weekend on-call rotations.
