How would you like to become a part of a team providing compassionate care to Veterans? This position is stationed at the Illiana VA Health Medical Center in Danville, IL. For this Whole Health position, it may be filled as a Social Worker, Registered Nurse (RN), Registered Dietician, or Psychologist. The Whole Health Partnership (WHP) Program Manager will report to the Associate Chief of Staff Ambulatory Care Service. The Whole Health Partnership (WHP) has full responsibility for the strategic planning, development, implementation, and ongoing evaluation of multiple programs aimed at pro-active, patient-driven, and patient-centered care and promotion of health and well-being at all sites of the Medical Center and four surrounding CBOCs as an integral component of the Patient Aligned Care Team, a multi-disciplinary Patient Aligned Care Team (PACT) consisting of providers, psychologists, nurses, and clerks, as well as in support of all other disciplines within the healthcare system. Provides certified health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing health ones. Contacts eligible participants, actively and passively, for enrollment and effectively explain the Whole Health process. Conducts interventions for Veterans using one-on-one or group modalities in outpatient and inpatient settings. Provides direct budgetary supervision of Whole Health Partnership (WHP) program Provides direct supervision of staff hired for Whole Health Partnership (WHP) program Provides education and experiential opportunities to engage Veterans in complimentary and integrative approaches, including but not limited to, guided imagery, meditation, mindful awareness, etc. Works closely with Veteran centered care coordinator, Health Promotion Disease Prevention, Health Behavior Coordinator, MOVE coordinators, Veterans Health Education, My HealtheVet, Inter-disciplinary Pain Rehabilitation program, Recreational Therapy, and employee wellness. Provides follow-up care through telephone clinics, VA video connect or secure messaging between face-to-face appointments. Actively guides Veterans in the exploration of core elements of personal health planning through individual and group-based programs. Encourages and assists Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the Veteran's goals. Establishes relationships with external local and national community service organizations supportive of mission of VISN, medical center, and PACT initiative.
