SUMMARY OF POSITION:
An opportunity exists for a Patient Access Coordinator in Population Care Management at Penn State Health.
As a member of a Care Management team, the Coordinator works as a liaison between individuals and the health care organization, with their primary role being to provide excellent patient service. Patient access coordinator fields phone calls from individuals, helping answer their questions by listening, educating and problem solving. As a member of the team, the Coordinator will be responsible for the daily operations and routing of communications between designated population to assure appropriate and timely coordination of services, including but not limited to medical service, access physician, service referral, and direction of services offered at the organization and community.
This is a full-time, day-shift position.
Manage inbound calls from individuals and referral sources; redirecting individuals as needed and performing all elements of individual engagement and needs assessment
Follows developed algorithms to determine next steps in referral process.
Demonstrates sound knowledge and judgement involving appropriate clinicians when presented with clinical concern from an individual.
Places outbound calls to support navigation to appropriate resources
Collaborates with the population health team and department leadership in the resolution of operational problems and aids with health promotion activities.
Adheres to all organizational policies and procedures
High school diploma or equivalent
Graduate of an accredited Medical Assisting (MA) Program or Licensed Practical Nurse (LPN) program required
Three years' of experience as a MA or LPN in an outpatient practice or clinical specialty area
Active PA license status as a Certified MA, Registered MA or LPN
Excellent oral communication skills and computer skills
Associates degree in Healthcare Administration or a related field
Prior Call Center, Healthcare IT, or Healthcare operational experience
Strong customer service background - Individuals in the patient access coordinator role must be able to quickly identify patient needs and work to resolve issues in a friendly, professional manner.
