Seeking a qualified registered nurse to work as a Dementia Caregiver Case Manager The Dementia Caregiver Case Manager (DCCM) is a RN responsible for providing education and support of the family caregiver of Veterans with Dementia and other Cognitive Impairments (CI) in the home through proactive care planning and utilization of VISN approved Dementia resources. He/She is responsible for providing support necessary to maintain the Veteran safely in the home environment for as long as possible. This position has responsibility for the : Receipt of referrals and initiation of assessment for the caregiver of an identified Veteran with dementia The development of a care plan with a focus on caregiver education, safety and effective strategies to assist in the management of the person with dementia Maintaining contacts at determined intervals with the caregiver and initiating additional medical or psychosocial supports as needed in the individualized situation Understanding the program office initiative REACH- VA Collaborative approach with the social worker in the dementia caregiver support program Thorough and accurate documentation of all information included in the educational sessions, treatment plan for the dyad ( Veteran and caregiver) The incumbent participates in: National Program calls and educational offerings The local dementia care committee Education to staff members in the facility regarding dementia recognition and management strategies Work Schedule: 8:00am-4:30pm Monday-Friday Telework: Ad Hoc Virtual: This is not a virtual position.
