Facilitates entrance of new patients into care programs. Serves as the liaison between the primary care team, patient, and specialists. Collaborates with patients, families, primary and specialty care providers in multiple practice sites to facilitate access to services specifically related to the patient's health care needs.
Acts as the coordinator to identify appropriate patients, opportunities regarding scheduling, and documentation.
Coordinates with the provider, patient and other health care team members to enhance access to specialty care.
Collaboration with primary care and specialty providers as well as the health care team to promote best practice by initiating pre-visit planning and preventive and specialty care education and scheduling services when appropriate.
Coordinates appointment scheduling, laboratory, and diagnostic tests as needed.
Maintains systems and processes to assure timely follow-up and surveillance of post-follow-up patients.

*This person should be self-motivated and directed and enjoy presenting to groups. They should also possess creative skillsets to engage and motivate participants (employees and patient/families and community members) in planning ahead for healthcare decisions (Advance care planning). This position works independently but is also part of a small team. Organization and outgoing personality to sell this type of planning is helpful. Clinical knowledge base as well as experience in advance care planning or serious illness care is helpful.

Graduate from Specialty Training Program-Nursing (Required), Bachelor's Degree-Healthcare Related Degree (Preferred)
