The Patient Safety/Quality Management Consultant (PSQMC) serves as an expert practitioner whose practice encompasses a wide range of responsibility and accountability. The PSQMC is an independent and self-directed professional who assesses, plans, implements, coordinates, monitors and evaluates the quality and safety of the delivery of care for the Eugene Health Care Center (EHCC), North Bend Care Clinic, and Brookings Care Clinic. The Registered Nurse is assigned to the Quality Management Service, Office of the Director. The PSQMC functions as a program manager and works collaboratively with the Patient Safety Manager. Responsible for the development and implementation of patient safety policies and activities developed by the National Center for Patient Safety (NCPS), and other agencies. The individual provides direct support for the development, maintenance, and operation for the patient safety process that includes all levels of staff and Veterans. The PSQMC will work closely with the Risk Manager, High Reliability Organization Lead, Infection Prevention and Survey Accreditation Manager to ensure that all aspects of patient safety are incorporated into policies, procedures, and action plans. The incumbent will work to identify patient safety issues using a variety of tools including reviews of the 24h report, JPSRs, root causes analysis, HFEMAs, and other methods. After identification the PSQMC will lead and/or support others to identify appropriate interventions and put those interventions into practice. The PSQMC will also monitor those interventions through various auditing methods to ensure sustainability. The incumbent will work closely with Service Chiefs, Managers, Supervisors, Officers and Directors to ensure that RVAHCS is becoming a high reliability organization. The PSQMC assumes the leadership role in facilitating regulatory compliance for internal and external oversight bodies including but not limited to (e.g. The Joint Commission (TJC), Commission on Accreditation of Rehabilitation Facilities (CARF), VHA Directives and Handbooks, etc.), Occupational Safety and Health Administration, Office of the Medical Inspector, Office of the Inspector General. The PSQMC leads and supports performance improvement initiatives and acts as a subject matter expert in data analysis and data display. Performs audits and monitors implementation of action plans within all levels and departments throughout RVAHCS, and creates professional reports to be used by executive leadership and VISN to assess organization performance.
