The initial cut-off date for referral of eligible applications will be September 15, 2021, with subsequent cut-off dates every Monday. Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis until position(s) are filled. The RN Case Manager will develop and coordinate seamless transitioning of patients for acute, chronic, high risk, high cost, and/or clients requiring a higher level of care management in the inpatient and outpatient setting with a focus on discharge planning needs and self-care management. The Case Manager will provide support at the unit level, the service level, and the organizational level. A key aspect of the role is utilizing a collaborative process to assess, advocate, plan, implement, coordinate, monitor, and evaluate health care options and services to meet the needs of the individual patient. The Case Manager will be the resource person and serve as consultant for problem solving in nursing and organizational issues. The Case Manager will serve as a representative on medical center and service committees. The Case Manager possesses intimate knowledge of lnterQual criteria, utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines. Demonstrates knowledge and ability to apply principles of evidence-based practice. Accountable for utilizing critical thinking skills and sound judgment to ensure positive outcomes, maximizing third-party collections, delivering quality patient care, and performance improvement activities. Other duties: Ability to provide education on disease processes, treatment process, medication and symptom management, preventative measures, and acts as a health coach proactively supporting patients to optimize treatments Track patients with complex care needs, those identified as high risk and follow up on clinic appointments Ability to monitor activities to ensure timely multidisciplinary review and coordination of activities necessary for development of treatment plans, protocols, and supportive procedures Manage empaneled population to include screenings, patient treatment, symptom management, administration of immunizations/vaccinations/infusions, and track all treatments Assist medical providers with coordination of extensive testing required for GI/Liver disease, consultation, and submission of referrals as appropriate to specialties for higher level care needs Work collaboratively with other services for timely scheduling, determining eligibility for supportive/community services Develop and maintain content expertise in clinical care through networking, national conferences, educational offerings Other duties assigned by management Work Schedule: 7:30 am to 4:00 pm, 8:00 am to 4:30 pm (subject to change based on agency needs) Compressed/Flexible: Not Available Telework: Not Available Virtual: This is not a virtual position.
