Wilkes-Barre VA Medical Center is recruiting for one (1) full-time, RN - Nurse Manager Connected Care. The position requires work at the Wilkes-Barre main facility and travel to CBOCs as needed. This Nurse Manager serves as the immediate supervisor for the Home Telehealth Supervisor/Lead RN, Telehealth technicians, LPNs, My HealtheVet Coordinator, My HealtheVet Assistant, Lead Telehealth Technician, Telehealth Clinical Technicians, Program Specialists, and administrative support staff. This Nurse Manager is directly responsible to the Associate Director of Patient Care Services. The Connected Care Program utilizes a collaborative and innovative approach to the delivery of healthcare services. This approach utilizes technology to increase access to mental health, primary care and specialty services, promoting therapeutic relationships, with the goal of improving health and well-being, outreach to the Rural and Urban communities and improving patient satisfaction. In addition, the Connected Care Program seeks to make the home into the preferred place of care whenever possible and practicable, which is especially important with the vast rural and highly rural catchment area served by VISN 4 Medical Centers and campuses. This Nurse Manager will use telehealth technology and MHV to improve Veterans' access to health care by matching clinical provider supply with demand, especially in rural areas where it is difficult to recruit providers, and by providing them access to their health care information. Through technology, patients can be seen in VA Clinics in remote areas throughout the nation by providers who are located at VA Medical Centers. The Nurse Manager Connected Care is the designated first-line supervisory authority, responsibility, and accountability for the 24/7 operations of their unit, ensuring that subordinate personnel provide timely nursing care which complies with generally accepted standards of clinical practice. This includes the authority to accept, amend, or reject the work of subordinates. Duties include but not limited to the following: - Planning work to be accomplished by subordinates, setting priorities and preparing schedules for completion of work - Assigning work to subordinates based on priorities, selective consideration of the difficulty and the requirements of the assignments, and the capabilities of subordinates - Evaluating the performance of subordinates - Making recommendations for appointments, advancements or reassignments of subordinates - Giving advice, counsel, or instruction to subordinate personnel on work and administrative matters - Hearing and resolving complaints of subordinates and referring more serious complaints not resolved to higher level supervisors - Recommending and/or taking disciplinary action where appropriate - Identifying developmental and training needs of subordinates and providing or making provision for such development and training. - Maintaining continuous accreditation readiness of his/her units. - Performance improvement data, customer service/patient satisfaction, patient and employee safety, interviewing applicants, employee relations, standards of care and practice, the work environment, and accreditation and regulatory standards. - Actively develops and manages collaborative partnerships with physicians and other clinicians. - Staffing their units on a continual basis (24/7), ensures staffing meets established nursing hours per patient day requirements, and works with other Nurse Managers to identify and share resources. - Time scheduling of the unit, coaches staff, deals with employee conduct and performance issues, completes recurring reports and performance evaluations in a timely manner. - Participation on service and medical center committees and task forces to improve nursing practice and patient care, identifies educational needs of staff and ensures that these needs are addressed. - Works in partnership with the union and adheres to EEO principles. - The Connected Care Manager will designate a cross-covering Nurse Manager in his/her absence to handle administrative and clinical issues, such as staffing coverage, granting leave, and patient and/or staff issues.
