SUMMARY OF POSITION:
The Director is responsible to direct all operational functions focusing on integration activities impacting transitional care, including collaboration with payers, care management, and quality teams across PSH. This role is also responsible for the direct supervision of providers working in the CPD Transitional Care area. All duties are carried out in collaboration with the physician leaders and administrative leaders in this area. The Director is responsible for maintaining quality service, financial stability, customer satisfaction, positive public relations, and expanding the program base. This position requires a high level of management expertise and excellent communication and interpersonal skills. This individual must be able to communicate effectively with leadership teams, care teams, support staff, and external stakeholders.
Bachelor's Degree or equivalent experience (4 years)
Seven (7) years of related experience in clinical operations, transitional care, or other related areas
Five (5) years of leadership experience
Must have experience with budgeting procedures
Must be able to develop an effective partnership with physicians and to communicate clearly and collaborate with all internal and external partners
Master's Degree preferred
