The TPS RN Care Manager will work in collaboration with interdisciplinary team members to assess and plan delivery of care or arrange services in their local community for veterans through a community provider. The TPS RN Care Manager works with both VA and non-VA inpatient psychiatric units to establish outpatient psychiatric care for veterans post discharge. The nurse will work directly with the team(s) psychiatric prescribers and other facility medical staff to ensure comprehensive care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
