The Outpatient Registered Nurse (RN) in the role of a Care Manager applies the principles of advanced clinic access and Patient Aligned Care Team (PACT). The PACT RN Care Manager works closely with the interdisciplinary team and performs care coordination activities in a clinic setting. This PACT RN Care Manager demonstrates leadership that is broad enough to improve the care for groups of patients, documenting and evaluating outcomes at the program or service level.
