Job Description:
Under limited direction and according to established policies and procedures, acts as a member of the care management team and department of Psychiatry and Behavioral Neurosciences treatment team by assessing, developing, implementing and monitoring a complete plan of care through the interdisciplinary team process in conjunction with patient and family in an internal setting.
- On a concurrent basis, assesses level of care, diagnostic testing and procedures performed, quality and clinical risk issues, and documentation for medical record completeness. Interacts daily with patient, family, treatment team, and other caregivers as necessary.
- Communicates with third party payers as necessary to review medical necessity of continued stay and collaborates with team to promote treatment in the least restrictive environment that is appropriate.
- Works collaboratively with team members to formulate, implement and evaluate educational strategies for patient and family.
- Analyzes outcome data to identify issues and make recommendations.
- Maintains current knowledge of health care and psychiatric techniques and practices by participating in educational programs.
- Maintains a working knowledge of the requirements of the payers most frequently seen with the patient population.
Qualifications:1. Graduate from an accredited school of nursing. Bachelors degree in Nursing or other health-related field, or equivalent combination of education and/or related experience. License to practice as a Registered Nurse in the State of Michigan. Or
2. Graduate from a Masters program in Social Work (MSW). Registration in the State of Michigan to practice as a Social Worker required. AND Three to five years clinical nursing or Social Work experience in behavioral health care. Previous experience with the targeted patient population preferred.
