Job Description Westmed Medical Group is currently seeking a full-time Weight Management Nurse Practitioner to work in collaboration with a physician on our Weight Management team primarily at the Purchase, New York location. Primary responsibilities include delivering high-touch, high-value assessment and care management services for comprehensive weight management in a multi-specialty group practice. Practice includes in office patient engagement, which allows for comprehensive patient centered care. Patient care and assessment in collaboration with the PCP and Multi-specialty care team including full medical history, medication review, Weight, Height, Waist circumference, Total cholesterol / LDL / HDL / triglycerides, Fasting glucose, Insulin, Hba1c, Systolic and diastolic blood pressure as well as behavioral and psychosocial assessment and use of data analytics to capture a comprehensive picture of the patient's health status and appropriate care planning and motivate change for desired outcomes.
This position also involves utilizing the latest medical advancements in weight management, identification of barriers to weight loss; including biological and medical factors, developing individualized, healthy and balanced eating plans, building a 'tool box' of skills for patients to attain long-term weight loss. Position provides guidance on shifting to more healthy behaviors (i.e., increasing physical activity), provides frequent monitoring to provide support and encouragement, oversight and engagement and coordinates associated resources and utilization between health care providers including behavioral medicine, physical therapy and bariatrics.
Note: This position requires frequent patient interaction in the physician practice setting and patient education at more than one location.
Essential Functions and Job Responsibilities:
- Assessing, diagnosing and treating the patient in the office
- Collaboration and communication with patient's primary care provider, specialist and interdisciplinary Weight Loss Team
- Referrals to specialists
- Referrals to community resources including home care, durable medical equipment & supplies and end-of-life and palliative care options
- Conducts Serious Illness conversations
- Ordering labs, tests and medications
- Medication reconciliation & adherence
- Prescription writing, orders, refills
- Patient & family education and support
- Discharge summary and transition back to the Westmed primary care physician
- Assess and interprets the need for and utilization of transitional resources
- Assesses and interprets patient needs, gaps, and requirements
- Solves complex problems and/ or conducts moderately complex analyses impacting weight loss
- Develops care plans for weight management and lifestyle change in collaboration with patient/caregiver/provider
- Works with minimal guidance, seeks guidance/collaboration on complex cases
- Translates concepts into practice
- Assesses the effectiveness and quality of services provided related to clinical, functional, and financial impact and outcomes
- Educates and supports patients to assist them in the best decision making for their health and wellness
- Collaborates on department specifics metrics
- Adheres to ethical, legal/regulatory and accreditation standards, licensing guidelines
- Other duties as assigned
Required Qualifications:
Education/Certifications/Licenses
- Licensed Nurse Practitioner in New York, dual licensure in Connecticut preferred
- Minimum of two years clinical experience.
- Bachelor's or Master's degree in Nursing
- Current BLS Certification
General
- Astute clinical skills required
- Self-directed worker
- Clinical competence in understanding
- Bilingual Spanish preferred
- Willingness to work alternate shifts - i.e., evening and some weekends
- Able to travel to any Westmed location
Technical skills
- Knowledge of obesity medicine, nutrition, bariatric surgery
- Knowledge of Preventive Care, Chronic Care and Population Health
- Knowledge of ACO, Patient Centered Medical Home (PCMH)
- Comfortable with data analytics as it relates to managing and improving patient care and outcomes
- Health coaching experience a plus
Social skills
- Communicate effectively with patients, insurance companies, and co-workers
- Communicate with MDs on a continuously basis in regards to patient care
Computer skills
- Must have strong computer skills and technical aptitude
- Knowledge of EMR, Microsoft Word/Excel, and Internal Medicine billing codes
