This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. The CCNM will ensure that subordinate personnel provide timely nursing care and coordination of services which complies with generally accepted standards of clinical practice. In addition to oversight of performance of the care coordination staff, s/he will also have primary responsibility to ensure the provision of case management of veterans identified as high risk for including but not limited to veterans with chronic disease, limited functional status, complex medical, psychosocial and/or mental health needs, severely-injured OEF/OIF/OND service members and veterans, and veterans with intensive continuing care needs that will require coordination of care and referrals for various levels of care including home and community for care, to appropriately coordinate home care services needs as well, and facilitate transfer to the VA facility as appropriate.
