This position will lead a team that focuses on assisting with discharging members from the hospital who are difficult to discharge from the hospital due to social determinants of health (SDOH). This will include assisting with administrative days requests from hospitals where a difficult to discharge member is no longer meeting inpatient criteria but is unable to discharge due to SDOH. .
Primary Responsibilities:
- Supervises and is accountable for professional employees
- Impact of work is most often at the local level
- Sets team direction, resolves problems and provides guidance to members of own team
- Work under guidance of CSM Associate Director of Medical/Clinical Operations) to set team direction
- Adapts departmental plans and priorities to address business and operational challenges such as staffing and the clinical model
- Influences or provides input to forecasting and planning activities
- Product, service or process decisions are most likely to impact multiple groups of employees and / or customers (internal or external)
- Staffing projections / future needs
- Contract compliance-audit responsibilities
- Inter dept. interaction-sales-PRS, Clin. Ops to ensure goals are met
- Ensure member transitions receive appropriate follow-up -
- Employee review process including MAP reviews, on-going performance evaluation / assessment
- Monitoring including but not limited to Learn Source program completion
- Local Training requirements and monitoring and addressing learning needs of the supervisors’ direct reports

Required Qualifications:
- Requires current unrestricted RN licensure in the state of Washington
- 3+ years of clinical experience in, acute care, , direct care or case management/discharge planning
- Computer / typing proficiency to enter / retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
Preferred Qualifications:
- 2+ years of experience in an acute hospital or SNF discharge planning.
- Experience in health care and insurance industry, including regulatory and compliance requirements
- Analytics/database experience
- Experience training staff
- Experience presenting in a group setting
- Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action
- Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others
- Demonstrated ability to assist with focusing activities towards a strategic direction as well as develop tactical plans, drive performance and achieve targets
- Proficiency in Microsoft PowerPoint
- Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action
- Ability to communicate complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others
