The community you’ll be caring for:

Clay County has relied on AdventHealth Manchester, formerly Manchester Memorial Hospital, in Kentucky since 1917. Today, our hospital in Clay County serves more than 60,000 patients every year, while staying true to our community-focused, patient-centric model of care.

The role you’ll contribute:

This position is accountable for the planning and implementation of patient care activities, coordinating the efforts of team members to achieve an individual Patient Care Plan, to maximize function within the home restoring health or assisting a peaceful death.

• Organize and implement individual patient care based on ongoing interpretation of physiological, psychological, sociological deficits affecting the state of health
• Coordinate the activities of health team members to provide comprehensive care and continuity of services and participate in team meetings

EDUCATION AND EXPERIENCE REQUIRED:

• Excellent oral and written communication skills
• Graduate of an approved school of professional nursing
• Maintain professional licensure of a Registered Nurse in the state.
• Current CPR certification
• ACLS
