Flex start time in morning : 8 hour shift between 8am -6pm

Atlantic Health System is seeking a Full-time Social Worker at Overlook Medical Center.

• Social Worker provides psychosocial assessments, counseling and facilitation of discharge plan in all areas within the hospital. Responsibilities include:
• Conducting psychosocial assessments
• Discharge planning for post acute needs
• Obtaining insurance authorizations
• Making referrals to appropriate community resources
• Active participation in family and team meetings
• Providing supportive counseling
• Crisis intervention
• Assessing and making referrals for abuse and neglect (adult and pediatric), providing support and counseling at the end of life

EDUCATION/LICENSURE:

* MSW with current LSW or LCSW license in the State of New Jersey

Relevant Experience:

* 1 year previous hospital SW experience and/or acute care experience preferred (knowledge of systems- financial, insurance, advocacy, legal, knowledge of community resources.)
* Familiarity with Epic preferred
