The role you’ll contribute:
The Care Management Supervisor is under the general leadership of the Care Management Manager or Facility Director and assists in monitoring and performing the daily operations and supervision of team members in the Care Management department. The Care Management Supervisor is responsible for the daily front line operations as assigned by the Care Management Manager or Facility Director. The Care Management Supervisor will serve as the primary point of contact for Care Managers, Social Workers and patients, to provide mitigation for any barriers/concerns that may arise on the inpatient units. The Care Management Supervisor will be responsible for supporting positive and safe patient outcomes, employee evaluations, and customer experience.

- Supervises daily operations of discharge planning and social services in the inpatient and emergency department setting as directed by Care Management Manager or Facility Director
- Assists Care Management Manager/Facility Director to implement the Corporate Care Management strategies
- Ensures active staff participation in daily Multidisciplinary Rounds
- Serves as a point of contact for all identified stakeholders
- Serves as a content resource regarding medical necessity criteria, patient status and discharge criteria
QualificationsThe expertise and experiences you’ll need to succeed:
- Registered Nurse: Associates Degree Nursing OR Social Worker: BSW
- State specific RN license required for Nurses
- Two (2) years of experience in direct Care Management
