Position Information:
The Community Case Manager and Advisor is a Social Worker responsible for providing case management to select patients in the community. Provides guidance and advice to patients and families in collaboration with an interdisciplinary team. Coordinates care in the community to optimize outcomes with allocated resources. Monitors the effectiveness of each patient's plan. Serves as a patient/family advocate and a communication link between the physician, patient, family, health care team, community resources, and payers to reduce redundancy and fragmentation of care. Anticipates potential delays in the health care process and acts proactively to avoid these delays. The Community Care Manager and Advisor will regularly work and visit with patients in their homes and living environments.
Experience Requirements:
2 Years Clinical - Case Management Or Related Position Experience Required
Education Required:
Master's Degree Social Work Required
Licensure/Certification Requirements:
Certified Case Manager Or Licensed Clinical Social Worker Preferred - And Maintain Vehicle Operator Insurability Required
