Job SummaryThis position is responsible to assure the care and coordination of HealthChoices members who have been classified as having special needs based on a non-categorical or generic perspective that identifies key attributes of ongoing physical, developmental, emotional, or behavioral conditions including, but not limited to: PHIV/AIDS, Children in Substitute Care, and Intellectual or developmental disabilities; managing Age-out, and the Lead program to ensure follow-up for children identified with elevated lead levels and other cognitive or congenital conditions that substantially limit typical activities of daily living.
