Additional Information About the RoleThe Community Health Manager position conducts the day to day operation of the Raising St. Louis program. The Raising St. Louis program provides free services to 21 zip codes in north St. Louis City and County. The manager hires, trains, and supervises a staff of supervisors, parent educators, intake coordinator, community health workers, community health coordinator, and licensed clinical social worker. The manager maintains relationships with our affiliates and community partners. Provides a vision for our community hub locations. Helps to plan and manage a program budget of $1.8M.

Role Purpose
Responsible for providing oversight, management and strategic direction over all aspects of Community or employee-based health programs. Serves as a central liaison and resource to community and BJC partners.
Responsibilities
- Develops and adheres to departmental staffing, revenue and/or expense budgets. Responds to changes in the business which may affect the ability to achieve the budget goals.
- Develops and oversees programs targeted for health literacy in the community and in collaboration with select external partners. May assist in management of the health of select patient populations through advanced health assessments, evaluations, diagnosis and interventions. Provides health education and information to patient populations within the community and/or BJC.
- Supports the hospitals in serving their community and/or employees with preventative and educational health services. Complements BJC and/or individual system hospital’s public health strategy by providing much needed services.
- Coordinates and/or implements BJC health literacy and screening initiatives for employees. Tracks, analyzes, and reports on relevant data. Assists in researching and planning new programs.
- Develops an annual comprehensive health plan for external clients as needed.
- Manages individual(s) including but not limited to: hires, trains, assigns work, manages & evaluates performance, conducts professional development plans. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.
- Seeks opportunities to pursue participation in research, planning and/or the evaluation of program components. Tracks, analyzes and reports on relevant data and disseminates findings appropriately. Implements findings and uses data to improve care of patients and the systems of healthcare delivery.

Minimum Requirements
- Bachelor's Degree - Healthcare Admin/related
- 5-10 years of experience
- 2-5 years of experience

- 10+ years of experience
- 5-10 years of supervisory experience
