Overview
St. Luke's Health System is seeking a Manager of Case Management to join our Health Partners team.
SLHP Care Management supports and educates patients in value-based payment models. The objective of care management is to provide patient centered care management utilizing best practices to deliver on the triple aim of better outcomes, lower costs, and increased patient satisfaction.
Responsibilities
- Provides day-to-day oversight of designated departments across multiple sites.
- Directs and guides staff in conducting assessments, interventions and developing managed care strategies and plans. Identifies opportunities to enhance and maximize discharge planning options for patients.
- Provides leadership, guidance, counseling and continuing education to staff. Will select, develop, coach, mentor, and assess performance of staff.
- Evaluates patient care data to ensure that care is provided in accordance with guidelines and organizational standards.
- Works collaboratively with interdisciplinary teams to ensure understanding and adherence to standards of practice and relevant policies and procedures for area of oversight.
- Serves as an expert resource to staff to assist with complex and/or difficult issues in area of oversight.
- Participates in budget development and fiscal accountability for areas of oversight.
- Perform other duties and responsibilities as assigned.
Qualifications
- Education: MSW.
- Experience: 6 year relevant experience in hospital case management, social work, or related field
- Licenses/Certifications: Licensed Clinical Social Worker (LCSW)
