Reliant Medical Group is seeking a compassionate and professional Registered Nurse Case Manager with flexibility and a creative approach to problem-solving to join their Care Management team in Auburn, MA.
This is a full-time position; Monday - Friday, no weekends, or holidays.
Provides comprehensive case management and care coordination for a diverse panel of patients with identified needs spanning age, condition, and population variables
“Coaches” (rather than “teaches”) patients to improve their health behavior to attain health-related goals, while working as part of a health care team (Providers, Nurses, Social Workers, Patients, Family, Caregivers, etc.)

Graduate from an accredited school of nursing
Active, unrestricted licensure as a Registered Nurse in Massachusetts
Current Massachusetts driver’s license with the ability to travel to hospitals, skilled nursing facilities, patient’s homes, and other sites where patients receive care as needed
Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Certification in Geriatric Nursing or Geriatric Case Management
5+ years of nursing experience, preferably with older patients
Proficient in computer use, the Internet and health information technology
Demonstrated excellent communication, interpersonal and organizational skills, with a flexible and creative approach to problem solving
Proven ability to work effectively both independently as well as a member of an interdisciplinary team
