The Pain Management/Behavioral Health Clinical Pharmacist Practitioner is a licensed pharmacist, preferably with specialized training or equivalent experience in pain management and behavioral health. The incumbent functions as a mid-level provider with prescribing privileges and works with assigned providers and health care team(s) in designing, implementing, monitoring, evaluating, and modifying patient pharmacotherapy with regards to pain management and behavioral health conditions.
