The Post-9/11 M2VA Social Work Case Manager provides comprehensive social work case management to Post-9/11 transitioning Servicemembers, Veterans (SM/Vs) and their families across the continuum of care and during the various phases of readjustment to civilian life. The social worker independently provides an array of social work interventions such as high-risk screening, psychosocial assessments, treatment planning, resource education, referrals, case management and care coordination. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
