Summary:
The Nurse Manager, Care Management is responsible for creating safe, healthy environments that support the work of the healthcare team and contribute to patient and family engagement.  The role is influential in creating a professional environment and fostering a culture where inter disciplinary team members contribute to optimal patient outcomes and grow professionally.  The nurse manager for care management are nurse leaders with 24-hour accountability and responsibility for care management processes across the ACH and ACHC continuum of care and provide the vital link between the administrative strategic plan and the point of care management. 
 
Responsibilities:
- Supervises employees, provides direction, coaches, trains, and develops and manages performance to company goals and expectations.
- Ensures the work environment is one that promotes employee engagement and retention and actively involves employees in work environment improvements.
- Participates in preparation of the department budget, manages and adheres to department budget to maintain expenditure controls.
- Responsible for performance improvement related to metrics, quality, patient safety, patient/staff engagement, regulatory and workplace safety requirements.
- Responsible for technology as it relates to care management, patient care and nursing practice
- Ensures the functioning of care coordination activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating).
- Implements mechanisms for identifying, assessing, and developing treatment plans for members with special health care needs.
- Develops and implements processes and resources for providing support to members who opt out of care coordination.
- Works collaboratively with other departments to integrate care management initiatives and goals with organizational programs.
- Strategic leadership responsibilities include change management, collaboration and shared decision making in an effort to implement and align unit vision with the organization's vision.

Other information:
Technical Expertise
- Experience working with all levels with an organization is required.
- Strong leadership skills including communication/organizational skills, time management, coping skills, motivation, problem solving, autonomy and supporting the team is required.
- Proficiency in MS Office, Outlook, Excel, Word, and Power Point is required.
- Budgeting and electronic time keeping software skills/experience is preferred.
- Relationship management and influencing behaviors including situation management, relationship management, influencing others, and promoting professional development.
 Education and Experience
- Education: Master's degree is required, MSN is preferred.
- Certification: Valid RN license and Basic Life Support are required.
- Experience: 5 years care management experience4. Years of experience supervising:  1 years  supervising professional level and/or professional level clinical employees is required.
