PRIMARY FUNCTION:Assumes primary responsibility for assessment, planning, development, implementation, and evaluation of Care Management Staff education and quality/safety initiatives. In addition, would assist in onboarding new IT initiatives to increase efficiency of the Care Management Staff and coordination of care to ensure safe and effective transfers in the movement of members across the care continuum. PRINCIPAL DUTIES AND RESPONSIBILITIESProvides leadership through the coordination and management of projects that support Care Management staff development and education initiatives in accordance with Care Management and CCHS philosophies and goals.
