Job Summary

This innovative nursing position will work directly with the local physicians, Nurses, and clinical team members to reduce hospital admissions by transitioning the patient's care to a home-based model of treatment. The patient will receive care in their home.

Job Duties

Position Details

• Hours: Day/Evening Shift. Hours will be between 8 AM and 6:30 PM with some flexibility in scheduling based on patient need
• Weekends: Every other weekend
• Holidays: Every other holiday
• On Call: Rotation based. Will be reviewed at the interview
• Travel: Company car will be provided. This position community based and requires travel throughout Luzerne and Lackawanna Counties.
• Required Experience: 3 years of RN work experience
• Preferred Experience: Acute care or inpatient nursing experience

Graduate from Specialty Training Program-Nursing (Required)

Experience

Basic Life Support Certification - Default Issuing Body; Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania
