The incumbent is an expert RN Care Manager within the Patient Aligned Care Team (PACT). Incumbent is a self-directed professional nurse who assumes a leadership role in the direct and indirect management of a group of patients through the coordination of care with physicians, nurse practitioners, medical residents/students, other health professional and hospital systems. The role resides throughout all sites within primary care nursing and reports directly to the site Nurse Manager.
