The VA Sierra Nevada Health Care System (VASNHCS) Transfer Coordinator/Inpatient Case Manager (TC/ICM) RN practices according to guidelines established by facility leadership for internal and external transfers of Veteran patients into and outside of the facility. This position serves as the point of contact and facilitator of inter-facility transfers. The TC/ICM RN also coordinates care across the continuum for a group of medical and surgical hospitalized patients to whom they are assigned. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
