St. Luke's Virtual Care Center is searching for an experienced RN to join our Remote Patient Management (RPM) team as a Nurse Supervisor.

Unit/Position Summary

Remote Patient Management (RPM) is a patient care program utilizing two way audio-video coupled with remote patient monitoring devices to help care for patients in their homes. It is staffed with experienced care managers and allied health professionals who utilize subjective and objective information received from in home telehealth systems to provide proactive and holistic care to patients. The RPM care model alerts our clinical staff to subtle deterioration in a patient's condition, allowing them to perform early interventions in order to avoid adverse outcomes. The RPM program is a new and rapidly expanding program which seeks to serve patients throughout Idaho. Advanced critical thinking skills, mature ability to form positive relationships with patients and a passion for innovative care models is essential.

The Virtual Care Team is located in Boise and operational from 7a-11p. This position will be 9 hour shifts, 4 days a week with variable hours based on department needs.

Responsibilities

• Provides day-to-day oversight of the unit/department(s) assigned.
• Provides leadership, guidance, counseling and continuing education to staff. Will select, develop, coach, mentor, and assess performance of staff. Works with staff to promote resolution of conflicts and counsels staff when clinical practice standards are not met.
• Assists with coordination of schedule to ensure adequate staffing levels.
• Collaborates with all members of care provision team to share knowledge and plan for optimal outcomes.
• Serves as administrative authority for the smooth function of all units on assigned shift.
• Exercises considerable judgment in applying professional knowledge in solving problems within established policies and practices. Serves as expert and resource to staff.
• Coordinates quality initiatives and process improvements.
• Assists with budget planning and clinical needs assessments.
• Perform other duties and responsibilities as assigned.

Qualifications

• Education: Bachelor's of Nursing Degree
• Experience: 3 years' relevant experience
• Licenses/Certifications: Current RN licensure from the State of Practice
