OverviewThe St. Luke's Health System Hospice team is looking for a full time RN Care Coordinator/Case Manager. Hours: Monday - Friday (8:30am - 5:00pm) with rotating evening call (5:00 pm-8:30am). The care provided by our Hospice team allows people to pass with dignity in their own homes, in familiar surroundings, with those they love by their side. The RN Care Coordinator works with physicians and multidisciplinary team members to develop and manage a plan of care for an assigned case load.
