The Association is seeking a Vice President, Population Health to join our team in Elkridge, MD. Reporting to the Senior Vice President, Care Transformation & Finance, the Vice President will collaborate closely with colleagues and hospital leaders to execute MHA’s advocacy goals for Maryland hospitals on issues related to care delivery transformation and population health. This position is responsible for working with MHA members (executive, physician, nursing and operations leaders) to lead population health improvement strategies under the Maryland Total Cost of Care Model and the new States Advancing All-Payer Health Equity Approaches and Development (AHEAD) Model that will launch in Maryland.
Essential Duties and Responsibilities
Create, shape, and advance evidence-based policy proposals for key health improvement strategies across the full healthcare continuum including primary care, chronic disease management, emergency care, and post-acute services.
Lead MHA member workgroups and learning collaboratives to identify conditions driving avoidable utilization, readmissions, and/or cost within the hospitals/health systems and support members in identifying solutions by providing data analysis, best practices, and research.
Represent MHA and its member hospitals/health systems in state agency led policy workgroups for population health planning and health improvement focus areas (e.g., maternal health, diabetes, behavioral health, etc.) and disseminate relevant information to member hospitals/health systems.
Establish and foster constructive working relationships with the Maryland Department of Health and Health Services Cost Review Commission staff in areas related to population health programming (e.g., “Revenue for Reform”, Regional Partnership Program, population health management reporting, etc.).
Monitor and maintain understanding of significant government, leading hospital, and think tank activities in value-based care, care transformation, population health, health disparities, and quality.
Assist in development of budgets and scopes of work for consulting support and direct the work of selected contractors.
Project manage and coordinate within and across teams to ensure MHA priorities are completed according to deadlines and with appropriate input from colleagues and hospital leaders.
Create documents such as slides, background memos, comment letters, and talking points that facilitate discussion among hospital leaders, clarify policy options or advocate on behalf of the hospital field. This requires the ability to synthesize, distill and communicate complex clinical and policy issues to a non-technical audience and communicate from an advocacy perspective.
Create and update work plans; execute on next steps identified during meetings; follow-up with individuals to understand and resolve concerns and barriers; accept and manage ambiguity and complexity in reaching objectives
Qualifications
Clinician Candidates
Physician (M.D. or D.O.) candidates should have a medical degree and experience in quality management, performance improvement, and/or care redesign.
Nurse candidates should have a Master’s degree in Public Health, Healthcare Administration, Nursing, or a related field with experience in quality management, performance improvement, and/or care redesign.
Minimum of 5 years of experience directing health programs with at least 2 years in a leadership in population health management.
Knowledge of CRISP, EMRs, data, technology, care management, analytics and other key population health functions.
Demonstrated success leading improvement initiatives
Complex problem-solving skills in individual and group settings
Experience and/or certification in project management, process improvement, or change management highly desirable
Experience managing health care projects, or familiarity with clinical or administrative operations, or subject matter including innovation models, care transformation or integration, clinical pathways, population health, patient engagement, quality, and health equity
Experience giving and receiving feedback on content in a collaborative environment, ability to manage multiple priorities, shift-gears and re-prioritize as circumstances change
Strong communication skills and the ability to interact with multiple internal and external audiences
This is a hybrid position with the ability to work virtually two days per week and work in our Elkridge, MD office three days per week.
