DescriptionWe are seeking a detailed-oriented Intensive Case Management Social Worker to join our team. Under the direction of the RN Assistant Director, you will manage and collaborate closely with UCLA Medicare Advantage senior patients admitted in the hospitals, SNF and in ambulatory setting, collaborate with Faculty Practice Group clinic, community resources, other providers, managed care operations, and the UCLA Health System. Your primary responsibility will be to implement, coordinate, monitor and offer options for very high risk patients with multiple comorbidities , ensuring their healthcare needs are met through effective communication and collaboration with providers, Intensive Case Management Team ,
