Job Summary
The Registered Nurse Case Manager Team Lead functions as a primary preceptor and is responsible for assisting Management and the Educator in developing and maintaining core competency of the Care Manager staff, participating in department curriculum and training in collaboration with the Educator. In addition, the Team Lead assists with department process improvement initiatives. The Team Lead assists in developing and mentoring case management staff. Collaborates with leadership, and departmental staff regarding changes that impact case management's role. The team lead will carry standard caseload, and provide significant input on complex cases. Leads performance improvement initiatives, and provides assistance and input on staff accountability and adoption of case management best practice. Ensures staff coverage, maintaining staffing schedules and time off calendars. Preps for meetings, and monitors front line staff data for performance and accountability. Can cross cover Care Manager positions across the care continuum as needed and required, including weekend rotation. Takes direction from the Management team to provide support to the department for day to day operations and will attend select meetings and represent the Case Management Department.
Minimum Job Qualifications
Licensure or other certifications: Registered Nurse with current Georgia license, and case management certification (e.g. CCM or ACM) within 6 months of hire/promotion, or first available testing date. Eligibility requirements for certification: 12 months full-time case management experience supervised by a certified case manager, or 24 months full-time case management experience not supervised by a certified case manager.
Educational Requirements: Bachelors Degree.
Minimum Experience: A minimum of five (5) years acute care case management experience. Experience as a preceptor, trainer or content developer in healthcare setting a plus; must be well versed in State and Federal Regulations.

Essential Tasks and Responsibilities
Develop and implement training program for Care Management Department. Develop, implement and update training resources.
Mentor and train new employees; directly supervise and collaboratively work with new employees during orientation period in conjunction with leadership team; and with RNCM as available to assist.
Develops weekly goals for new employee and monitor/evaluate their performance during and after orientation period.
Coaches employees when necessary regarding performance concerns and provides additional education or training as appropriate. Provide follow up training and/or mentoring on a quarterly basis during their first year of employment.
Work collaboratively with others to develop and implement training program/updates for Case Management staff; collaborates with various internal stakeholders in the design and development of orientation program, along with evaluation of training effectiveness.
Provides input and direction on complex Cases and may handle a select few cases at the direction of the Manager or Director, and provides staff accountability on these and all cases, monitoring progress.
Provides day to day support to the Management team for Care Management operations, including staffing, performance improvement initiatives and staff accountability.
Assess organizational and departmental needs related to context development. Provide guidance to CM department on presentation of educational and training material.
Anticipate, manage and resolve/escalate concerns regarding staff development with leadership support.
Maintains focus on team development, standard work and on-going competencies for the CM staff.
Monitors staff level accountability data and provides input and 1:1 coaching for those not meeting requirements.
Leads process improvement initiatives.
Provides significant input and direction on complex cases, and may take the lead on some cases at the request of the Manager or Director.
Maintains staffing schedules, and time off calendar, ensuring adequate coverage.
Assists Management in the day to day operations of running the department, as requested and directed.
Assist in department wide systems training/updates on a quarterly basis to ensure staff are performing daily functions efficiently. Develop and implement competency checklists to ensure staff follow departmental processes or guidelines, and make contextual modifications as needed.
Consistently demonstrates a 'sense of urgency' in his/her work, while mindful of the pillars and financial stewardship opportunities.
