SUMMARY OF POSITION:
Responsible for developing and implementing network and performance strategies to advance Penn State Health?s mission, as being a value provider. Serve as the primary liaison between the organization and external entities, ensuring alignment with performance goals related to quality, cost, and efficiency. Act as a central point of contact for fostering collaboration, driving accountability, and improving care delivery processes. Support the standardization of workflows and initiatives to enhance patient outcomes and reduce unnecessary costs. Align operations with value-based care goals to improve financial performance and organizational success. This role will be responsible for leading and managing initiatives that improve quality care, patient safety and clinical outcomes through process improvement. Network and performance strategies are focused on the post-acute care facility continuum of care including but not limited to skilled nursing facilities (SNF) and inpatient rehabilitation facilities (IRF), hospice and home health agencies (HHA).
MINIMUM QUALIFICATION(S):
Bachelor's degree
Minimum three (3) years of related experience required
Project Management experience
PREFERRED QUALIFICATION(S):
Master's degree or higher preferred
Provider/Healthcare network development
Strong skills in network and business development, managing the relationship of health care business across various care settings
Work history in skilled nursing facilities (SNF), inpatient rehabilitation facilities (IRF), hospice and home health agencies (HHA).
