Overview Delivers top quality care management services to identified patient populations through working collaboratively with the multidisciplinary team. Advocates, guides, and intervenes on behalf of patients, families, and/or caregivers. Engages identified patients in appropriate care management programs and provides patient centered interventions. Strives to positively influence the overall patient health outcomes as measured by clinical quality and utilization measures. Supports the achievement of system-wide population health goals within the framework of continuous improvement methodologies.Associate's Degree in Nursing required.
