Qualifications:
- Board-certified Hospice & Palliative Care Physician
- Minimum 5+ years of post-fellowship experience
- Ability to obtain a Wisconsin license
This Medical Director role supports inpatient, outpatient, and home-based community visits across Fond du Lac, Ripon, Waupun, Baraboo, Columbus, Madison, Janesville, and Monroe. The position partners collaboratively with 2 Physicians, 11 Advanced Practice Providers, and 15 dedicated support staff, including Registered Nurses, Social Workers, Chaplains, and Medical/Scheduling Office Assistants, delivering comprehensive, patient-centered care across the Wisconsin Region.

- .2 FTE Palliative Medical Director duties, .2 FTE Hospice Medical Director duties, and .6 FTE dedicated to Palliative Care Outpatient and Telemedicine Care
- Collaborating provider for Hospice & Palliative Care APPs
- Lead program initiatives
- Set quality and safety metrics
- Interdisciplinary team – Collaboratively work with Administrative Leaders, Clinical Staff, and Service Lines
- Help develop and grow the Regional Hospice & Palliative Medicine Program
